We are currently seeking a Licensed Social Worker (LSW) to serve as our Director of Social Services and lead with heart in our mission-driven community.
About the Role:
The Director of Social Services plays a vital role in supporting the emotional and psychosocial well-being of our residents and their families. This leader collaborates closely with the interdisciplinary team to ensure that care plans are person-centered and promote dignity, independence, and quality of life.This position requires an active LSW (Licensed Social Worker) license in the state of Ohio.
Key Responsibilities:
- Conduct pre-admission interviews with prospective residents/guarantors and explain center services.
- Partner with the Director of Nursing to evaluate patients prior to admission.
- Facilitate the admission process and document all forms and agreements.
- Assess resident needs and develop individualized social service plans.
- Provide emotional support to residents and families, including crisis intervention.
- Maintain relationships with outside agencies to coordinate resources and services.
- Plan and deliver staff in-services on the social aspects of long-term care.
- Coordinate discharge planning and continuity of care services.
- Maintain accurate and confidential social service documentation and resident care records.
- Participate in care plan conferences and departmental meetings.
- Engage with the community through outreach and professional development activities.
