at Thome PACE works in a close collaborative relationship with the
Providers and Registered Nurses to provide integrative care to
frail elders with complex needs including, but not limited to wound
care, medication administration, participant and caregiver
education, and service coordination. The LPN utilizes a systematic
approach to practice incorporating critical thinking, planning,
implementation and evaluation of outcomes to pro-actively manage
and advocate for PACE participants. The focus of care provided is
to enhance functional capacity, encourage autonomy, and assure
coordination of care and services while monitoring participants
progress toward care plan outcomes with charting and documentation
are performed in accordance with Thome PACE policies and
procedures. Driving is required within the Thome PACE service area
along with flexibility in hours to coordinate care with participant
family or caregivers after center hoursSkills and Knowledge--- Able
to read, write, speak, and understand the English language.---Must
possess the ability to communicate tactfully with participants,
family members, visitors, team members and external
agencies.---Function independently and have flexibility, personal
integrity, and the ability to work effectively with older adults,
personnel, and support organizations.---Meet a standardized set of
competencies for the specific position description established by
Thome PACE and approved by CMS before working independently.The

description.Required Qualifications:--- High School Diploma or GED
required--- Licensed Practical Nurse with current Michigan
licensure.--- BLS for healthcare providers or CPR Certification---
Minimum of one (1) year working with the frail and elderly. ---
Must possess a current State of Michigan driver's license and
maintain an acceptable driving record--- Must have reliable
transportation available on a daily basisAll employment offers are

physical, and TB Screen at time of hire. In addition, proof of
COVID-19 vaccination or approved exemption is required.Desired
Qualifications:---Must have three (3) years' experience as a
