Work Schedule: Candidate needs to be flexible with work schedule
based on business needs.
Location: Lansing, MI
The LPN/LVN Urgentivist Extender conducts in-home/field-based
visits to address urgent clinical issues with Landmark patients.
While on-site, the Field Nurse will evaluate patients and
communicate, via telephone or secure video, with the supervising
Landmark provider. The provider will direct the evaluation and care
provided. All activities will fall within the employee's scope of
practice as set by the State they are practicing in.
Primary Responsibilities:
Collect Patient Information: Vital signs, physical exam, point of
care testing (E.g. - glucose levels), venipuncture, Kardio Mobile,
medication compliance and reconciliation, adherence to discharge
instructions, etc.
Patient treatment and procedures: Skilled at phlebotomy and IV
placement and therapy, Catheter care and replacement. Administering
nebulizer treatments, and teaching proper protocol, wound care,
medication administration, etc. Treatment directives are only per
the supervising Provider's direction
Patient education: Common chronic conditions and education topics
include congestive heart failure, COPD, diabetes, hypertension,
fall risk assessment, overall health literacy per the direction of
the Provider
Provider collaboration: Skilled at concise and articulate
communication to remotely discuss evaluation and treatment with
supervising Provider. This collaboration will occur during every
patient visit unless the Field Nurse is following an approved
Landmark protocol (e.g., Post-Hospital Visit).
Care coordination: Schedule follow-up telephonic or in-home
interactions as needed
Documentation: Assist supervising Provider with documenting
findings and outcomes of the visit
Partner with Landmark interdisciplinary team (providers, nurses,
social workers, etc.) to drive improvement in patient outcomes and
satisfaction
Actively participate in weekly interdisciplinary team meetings
Conduct home visits and/or phone or video calls to patients to
collect clinical and psychosocial information, and carry out
designated tasks, including but not limited to:
Resource referral and connection
Quality measure documentation, support, and follow up
Telemedicine facilitation and set up
Collect information regarding patient conditions using standardized
checklists and templates

Required Qualifications:
High School Diploma / GED (or higher) with Current LPN license
Certified in CPR
Ability to work a variety of shifts
Comfortable working with electronic medical records and mobile
technology
Comfortable with conducting independent home visits
Knowledge and proficiency of basic Word, Outlook, and Excel
applications
1+ years of experience evaluating, assessing, and providing patient
care
Valid Driver's License and insurance with access to reliable
transportation that will enable you to travel up to 75% to visit
clients and / or patient sites within a designated area and you
must comply with all the terms of the Optum Motor Vehicle Safety
policy
Full COVID-19 vaccination is an essential job function of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

Preferred Qualifications:
Experience in rapid evaluation and treatments in home setting
AA degree or higher
Experience with Catheter care, replacement, and phlebotomy
Urgent care, emergency room, hospice, geriatric care, or community
home health experience
Physical Demands:
Regular requirement to sit; use hands to touch, handle or feel
Regular requirement to talk and listen
Regular requirement of bending
Occasional requirement to stand; walk and reach with hands and
arms
Occasional requirement to lift and/or move up to 10 pounds
Vision abilities include close vision, peripheral vision, depth
perception and ability to adjust focus
Ability to travel frequently by car
Soft Skills:

Excellent communication and interpersonal skills, with a strong
emphasis on empathy and building rapport with patients
Work Environment:
Initial high frequency visits to patient residences followed by as
needed visits during market implementations
Will require regular travel to patient residences and
non-home-based market offices
Noise level is usually moderate
