Job Description:

General Summary: The hospital liaison works directly with the inpatient psychiatric hospitals to coordinate appropriate discharge and aftercare planning. Ensures individual receive timely and appropriate care to prevent recidivism. Incorporates LifeWays' mission, vision and values into decision making. Essential Functions:

  • Acts as a member of the Crisis Team.
  • Participates in the treatment team for individuals receiving inpatient treatment, participates in discharge planning, and coordinates aftercare appointments.
  • Assists in the development of person-centered plans and monitoring of consumer progress.
  • Acts as a liaison with the community to ensure continuum of care and support for consumers.
  • Develops, implements, and maintains tracking mechanisms associated with inpatient admission and coordination of discharge planning as requested.
  • Completes intervention documentation according to agency guidelines and standards as well as in accordance with funding source regulations.
  • Will refer individuals in a timely manner to the appropriate health practitioners for assessment, person-centered planning, and/or supports and services; or if the individual is not eligible for PIHP or CMHSP services, to community resources that may meet their needs.
  • Inform individuals about all the available mental health and substance abuse services and providers and their due process rights under Medicaid, or MIChild, Healthy Michigan Plan, SABG, and the Michigan Mental Health Code.
  • Performs all duties as assigned to ensure the clinical care division runs efficiently and effectively. Duties include but are not limited to: Triage, Open Access, Rapid Access, REMI screenings, Call Back Que, Waitlist updating, specialty screenings (M-CHAT, SCQ, etc), testing (ADOS/ADIR), petitions, brief screens, intakes, pre-screens, crisis and safety planning.
  • Duties are performed in a variety of settings to include but not limited to: the office, community locations, emergency departments, schools, consumer homes, and jails.
  • Work a non-traditional schedule which may include evenings, weekends and/or holidays.
  • Provide on-call services, as needed or assigned. Required Education:
  • Master's degree in Social Work, Counseling, Family & Marriage Therapy or Psychology, required.
    • Hospital Liaison I: Master's degree required. Limited Licensed Masters Social Worker (LLMSW), Limited Licensed Professional Counselor (LLPC), Limited Licensed Psychologist (LLP)
      • Hospital Liaison II: Master's degree required. Fully Licensed Masters Social Worker (LMSW), Fully Licensed Professional Counselor (LPC), Fully Licensed Psychologist (LP)
      • Hospital Liaison III: Master's degree required. Fully Licensed Masters Social Worker (LMSW), Fully Licensed Professional Counselor (LPC), Fully Licensed Psychologist (LP) and is a Certified Alcohol and Drug Counselor (CADC) or is a Certified Advanced Alcohol and Drug Counselor (CAADC).
