Clinic LPN

Status: Full-Time, Non-exempt
Clinic Hours: Monday-Friday, 8:00am-4:30pm
with on-call rotation

  • Annual Increases Job Summary:
    The LPN at Thome PACE works in a close collaborative relationship with the Providers and Registered Nurses to provide integrative care to frail elders with complex needs. The LPN utilizes a systematic approach to practice incorporating critical thinking, planning, implementation and evaluation of outcomes to pro-actively manage and advocate for PACE participants. The focus of care provided is to enhance functional capacity, encourage autonomy, and assure coordination of care and services.
  • Skills and Knowledge
    Must be self-directed, flexible and committed to the team concept.
    Able to read, write, speak, and understand the English language.
    Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
    Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations

    Required Qualification:
    High School Diploma or GED
    CPR Certification
    State of Michigan driver's license and maintain an acceptable driving record
    Minimum of 1-year experience with the frail and elderly or long-term care population.
    Licensed Practical Nurse with current Michigan license
