Job Description:
The Patient Placement Coordinator assesses admissions and
transfers for eligibility and makes bed placement based on the
patient's clinical needs. Under minimal supervision, coordinates
the placement of all admissions (elective, urgent, emergency) and
transfers in accordance with clinical standards and guidelines.
Evaluates patient in a clinical setting, with or without physician,
as required. Utilizes established criteria to recommend appropriate
level of care throughout the continuum of care. The Coordinator
using approved clinical screening criteria reviews and screens all
proposed and actual admissions for appropriateness of services and
setting, determining if the patient is a candidate for observation
or inpatient status. The Coordinator acts as a patient advocate and
communication link with other health care departments and community
resources. Coordinates bed utilization across all hospital patient
care units, Emergency Department, and all internal and external
points of entry. This position is responsible for ensuring that the
patient is admitted to the most appropriate bed/unit available.
EDUCATION/EXPERIENCE REQUIRED:Graduation from an accredited school
of nursingEnrollment in a Bachelor of Science in Nursing (BSN)
program within 3 years of hireCompletion of a Bachelor of Science
in Nursing (BSN) within 5 years of hire Two (2) year's work
experience in an acute care settingEffective written, verbal and
interpersonal communication skills
CERTIFICATIONS/LICENSURES REQUIRED:Current Michigan Nursing
license
PREFERRED QUALIFICATIONS:Previous experience with bed
placementBachelors Degree in Nursing (BSN)Professional
Certifications to support a culture of nursing excellence
