Registered Nurse SupervisorAmeriCare Medical provides integrated

to hire a full-time Registered Nurse Supervisor to lead our QCN
Home Health Care Medicare nursing team at our corporate offices in
Troy, MI. This person will report to the Director of Nursing and
will be responsible for the day-to-day care of our external patient
base while working closely and communicating with our internal and

DiscountEssential Job Duties and Responsibilities:Conducting
OASIS-D patient health assessmentsDeveloping and reviewing nursing
care plans that are individual to each patient per Medicare
guidelinesProviding care according to the care plan ensuring that
the emotional, physical, comfort and safety of patients are met to
the greatest possible extentPerforming routine nursing duties such
as providing care to patientsObserving and documenting patients
conditions in software system (Alayacare)Understanding and
implementing infection prevention practicesNotifying the Director
of Nursing of any health and safety risks or concernsMaintaining
confidentiality of client and corporate informationCompleting other
tasks as requestedRequirementsActive RN License in the State of
MichiganPrevious Medicare home care experience (3-5 years)Able to
start intravenous lines, training can be providedSynergy EMR
experienceOASIS documentation experienceCase Management experience
preferred but not requiredWork location varies between Troy office
and patient homes, as neededReliable transportation requiredAble to
use medical software in the home via tablet and/or smart
phoneVaccination verification of COVID-19 (preferred)This is a
full-time, 8:30 am - 5:00 pm, Monday - Friday salary position;
