Minimum of 1 year of experience required. Experience in adult home
health required. Epic experience required. License: Active RN
license required. Certifications: BLS certification required.
Must-Have: Proficiency in PICC, Port, trach, Pleurx, and patient
education. Familiarity with Medicare guidelines related to home
care. Ability to complete OASIS documentation is required.
Description: Position is for a Home Health RN. Shift will be from
08:00 to 16:30. Every 5th weekend is required. No float or call
required during shifts. Caps on daily patient caseload range from 5
to 6 visits per day. Required tasks include home infusion, urinary
catheter management, wound care, tracheostomy care, and clinical
assessments. Handling of adult and geriatric patients is necessary.
Staff will provide interdisciplinary support including physical
therapy and social services. Documentation must be completed using
the Epic charting system. All required skills and tasks are to be
performed in accordance with established guidelines. 5 Benefits of
