Job Description MANDATED Job Requirements - TRULY Board Eligible (within 5 years of eligibility with plans to test) or Board Certified in Family Medicine (will not accept Internal Medicine) - REQUIRED - OH license (or willing to obtain) - REQUIRED - NPI and/or License Number - Malpractice and/or Background Documentation - Date of agency's last communication with provider (note in "Additional Submission Details") - REQUIRED - Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED - Provider's preferred location and schedule option (note in "Additional Submission Details" field) - REQUIRED - Clean malpractice and license - REQUIRE - Compensation & Benefits: - Base Salary: $300 K - $400 K - wRVU bonus Sign-on bonus - Loan assistance (stipend options) - CME stipend relocation support - Full insurance (Health, Dental, Vision) - Malpractice coverage, PTO, Birthday Pay - PSLF-qualifying employer - Tuition reimbursement, EAP, career advancement - ?
