Full time Shift: Days (United States of America) Job Summary: As
the Clinical Pharmacist for ProMedica Pharmacy Medication
Management , you will be dedicated at the Health & Wellness Center
to do Transitions of Care. This pharmacist will see patients after
they are discharged from the hospital to reconcile meds, make
appropriate changes, ensure patient
understanding/access/affordability. In this role, you will manage
referred patients’ medication therapy and communicate with patients
and other pharmacists. This position will perform ambulatory care
clinical pharmacist functions and must be willing to cover at any
ProMedica Pharmacy Medication Management site as needed. You will
contribute to the education of pharmacy residents, students and
medical students as needed. The above summary is intended to
describe the general nature and level of work performed in this
role. It should not be considered exhaustive. SCHEDULE Monday-
Friday 8am - 4:30 pm half hour lunch at noon Possibly holiday,
on-call REQUIREMENTS Doctorate of Pharmacy (PharmD) C ompleted a
PGY-1 (Post Graduate Year 1) pharmacy residency (for all new hires)
or at least two to three years of hospital pharmacy experience (for
any previous hires) R egistered pharmacist (RPh) licensed in state
of practice . Must obtain other state license within 1 year of
